PASSPORT SERVICES
New Applicants only
(form DS-11) -All new applicants must appear in person -No cash or
credit/debit card payments. The only acceptable forms of payment are checks and
money orders.

No appointment
necessary and applications are accepted Monday-Friday between the hours of
8:00am and 3:30pm.
Renewal application forms are available at Plymouth City Hall, but applicants
must mail in their own passport renewal applications.

A Passport
Book is valid for travel internationally by air, land or
sea. A Passport Card is a limited-use, wallet-size
card, good for land and sea travel (not air travel) between the US and
Canada, Mexico, the Caribbean region and Bermuda. The application process and
form are the same for both the Passport Book and Passport Card.

We do not take passport photos.

Any drugstore, Costco, Kinkos or AAA office will accommodate
your needs for passport photos. Snapshots, vending machine photos,
school pictures, and full length photos are unacceptable.

CITIZENSHIP*
Previous US
Passport, ORIGINAL Birth Certificate** or Certified Copy with a raised seal, or
Certificate of Naturalization. Photocopies will not be accepted.

*If a customer is traveling within 14 days due to an emergency or other urgent
travel needs, he or she will need to make an appointment to appear at a
Regional Passport Agency. Appointments at Passport Agencies can be made through
the National Passport Information Center (NPIC) at 1-877-487-2778 (toll-free)
or 1-888-874-7793 (TDD/TYY). The automated telephone appointment system is
available 24 hours a day, 7 days a week. The applicant must pay the Passport
Fee, Execution Fee and Expedite Fee to the U.S. Department of State.
Application for Passport-Complete IN FULL-Form DS-11 in BLACK INK ONLY-
no white-out or items crossed out. Do not sign application until instructed by
agent. Incomplete submissions will delay passport issuance.

MINORS:

Applicant(s) must provide: Form DS-11, original
Birth Certificate, previous passport (if applicable) parents driver’s license,
passport photo, check or money order.
If one parent is unable to attend, a Form DS-3053, “Statement
of Consent” form can be completed and notarized for absent parent. Also
required is a copy of the absent parent’s drivers license FRONT AND BACK.
Children 16 or 17 must be accompanied by one parent.
Applicant(s) must provide: Form DS-11, previous
passport (if applicable), original Birth Certificate, drivers license or school
id, ONE parent’s copy of driver’s license, passport photo, check or money
order.
