Covid Vaccine Clinic

In a continuing effort to do our part to stop the spread of Covid-19, the City of Plymouth is offering two Moderna clinics this week, on May 13 and 14 from 10 a.m. to 5:45 p.m. at the Plymouth Cultural Center, 525 Farmer.

Those who received their first Moderna shot on April 15 are already scheduled to have their second shot on  Thursday, May 13. Those who received their first shot on April 16 are scheduled for Friday, May 14. Appointments are at the same time as your first shot.  Make sure to bring your vaccine card.

Walk-ins will be accepted for either first or second shots, however, if you receive a first shot at this clinic, you will need to make arrangements with Rite Aid for your second shot because the City of Plymouth does not have another clinic scheduled at the time you'll need it. 

Whether this is your first or second shot, please complete the consent form and bring it with you.
